While hoping to tackle epidemics with the light of science, I found myself repeatedly telling people that head-to-toe soup is not a medicine. I would like to begin this article by addressing the gaps in information about COVID-19 and explaining its most general characteristics, which are unfortunately rarely discussed on television. I hope that as you pour vinegar into your homes, you will keep these facts in mind. (By the way, vinegar has no benefit in preventing COVID-19.)

What is a pandemic? What is a pandemic disease?

A pandemic disease is an infectious disease that emerges simultaneously and widely across the world, affecting a large number of people. The public health problem that arises on Earth as a result of this is called a ‘pandemic’.

A pandemic is declared by the WORLD HEALTH ORGANISATION, and this declaration takes into account two fundamental characteristics: -the transmissibility of the agent -the virulence of the agent; in other words, its ability to cause disease.

In this context, Covid-19 has been recognised as a ‘pandemic’ by the WHO.

What is the cause of Covid-19?

First, on 31 December 2019, the WHO China Office reported cases of pneumonia of unknown cause in Wuhan. On 7 January 2020, it was determined that the cause of these cases was a coronavirus.

In fact, humanity had previously encountered the coronavirus family through SARS and MERS; however, this time, we are faced with another coronavirus from the same family that spreads more rapidly than either of those. This new virus was named SARS-CoV-2 due to its similarity, and the disease it causes is called Covid-19.

How is Covid-19 transmitted? First, on 31 December 2019, the WHO China Office reported cases of pneumonia of unknown cause in Wuhan. On 7 January 2020, it was determined that the cause of these cases was a coronavirus.

In fact, humanity had previously encountered the coronavirus family through SARS and MERS; however, this time, we are faced with another coronavirus from the same family that spreads more rapidly than either of those. This new virus was named SARS-CoV-2 due to its similarity, and the disease it causes is called Covid-19.

How is Covid-19 transmitted?

The disease is primarily transmitted from person to person via droplets. Particles expelled by infected individuals can enter the respiratory tracts of others directly through actions such as sneezing or coughing. Additionally, these particles can be transferred to the faces and, consequently, the respiratory tracts of others through touch.

It is possible for individuals who are infected but asymptomatic to transmit the disease in the same manner.

The disease agent is not very resistant in the external environment and is thought to lose its activity on inanimate surfaces within 2-3 hours. However, the humidity and temperature of the external environment, the amount of virus released, and the texture of the contaminated environment are also important factors that can extend or shorten the duration of virus activity.

What are the symptoms of Covid-19?

Covid-19 is a systemic disease, and could even be described as a haemoglobinopathy. Three types of response emerge in individuals exposed to the pathogen. A large group of patients overcome the disease with very mild symptoms, almost without realising they have fallen ill. The second group of patients experience fever, cough and muscle pain. When symptoms remain at this level, the diagnosed patient is treated and placed under isolation/observation at home for 14 days. The third group consists of patients considered high-risk. High-risk patients are older individuals with additional conditions such as hypertension or diabetes. In these patients, shortness of breath is added to the symptoms listed above. Despite treatment, deterioration in the patient’s general condition, the person’s own immune system cells fighting the virus going haywire, and the phenomenon known as a cytokine storm occurring are possible in this patient group. If this possibility occurs, the person’s lung function may decrease and even stop, and multiple organ failure may also occur.

The WHO lists the following as symptoms of Covid-19: fever, cough and shortness of breath, chills, shaking, muscle pain, headache, loss of taste and smell, and sore throat. We continue to learn more about the virus, which emerged just 3-4 months ago, and our experience is growing. Indeed, in field practice, findings related to the gastrointestinal system, such as thromboembolic events and diarrhoea, are encountered in addition to these.

How is a COVID-19 diagnosis made?

Three important points are evaluated when making the diagnosis:
1) PCR Test
2) Computed Tomography
3) Laboratory Tests – CRP Ferritin, etc.

The PCR test is controversial here. There is no doubt that PCR is capable of detecting the disease. The problem is that it is a test with a high false negative rate. The reasons for this include: 1) taking too little of a sample from the patient, 2) taking the sample too early or too late in the course of the disease, 3) the sample not being taken or sent properly, and 4) problems inherent in the test itself, such as PCR incubation or virus mutation. Therefore, even if one or more negative results are obtained from the PCR test, the possibility of Covid-19 cannot be ruled out. However, it should also be noted that this test is the only means of detecting the disease using samples taken from the body.

If the patient’s clinical presentation and, in particular, CT scan findings are consistent with Covid-19, even if the PCR test result is negative, the patient is currently treated as if they have Covid-19.

Unfortunately, in our country, diagnosis is only confirmed by PCR testing. While this may not cause problems when treating patients, statistically it can present a false positive picture, leading to delays in taking epidemic measures or premature lifting of measures, either abruptly or gradually.

Treatment and prevention

There is currently no specific treatment for the disease. Unfortunately, drinking head-to-toe soup, unless you feel like it, does not serve a therapeutic or protective function. But enjoy it anyway. Returning to medicine, certain groups of drugs are used in specific stages of the disease, both worldwide and in our country, and positive results are being achieved in treatment. However, as with the plague, malaria and tuberculosis, there is no drug yet that directly eliminates the causative agent. Both vaccine and drug research is continuing at full speed worldwide.

Promising studies are also being conducted in our country; however, there are also many discouraging studies that have been terminated. The novelty of Covid-19 disease requires time for specific treatment and vaccines.

At this point, it is essential to be well aware of the ways to protect oneself and use them correctly in order to minimise the damage caused by the disease. Preventing contact, maintaining physical distance, paying attention to hand hygiene, and wearing masks in outdoor environments for both patients and healthy individuals are among the most important measures to prevent the spread of the disease.

Reducing human mobility, especially in large cities, is one of the safest methods to limit the disease.

In this context, a decision by authorities to impose a 14-day or longer lockdown will slow down and suppress the disease. Failure to take this measure for economic reasons will result in the application of the hidden herd immunity method in the long term, which could be more severe than the economic losses that are likely to be incurred. Herd immunity always comes at a price, and it is often disadvantaged individuals who pay that price. Furthermore, it should not be forgotten that relaxing measures for people over 65 and under 20, unless absolutely necessary, could lead to dangerous consequences, especially considering the risks faced by people over 65.

I believe Covid-19 will not be the last pandemic we encounter. Throughout their existence, humans will continue to encounter natural or artificial viruses and disease agents. Even if brought under control, Covid-19 does not appear to be disappearing from our lives anytime soon. However, progress will undoubtedly be made in its treatment, a vaccine will be found, and advances will be made in prevention methods. This pandemic experience, especially if we recall the currently accepted animal-to-human transmission scenario in the emergence of the disease, may prompt us to question our lifestyle practices and perhaps adapt them to be more in tune with nature, as well as to establish a more altruistic world order.

Since COVID-19, coronavirus, and bat soup disease entered the Turkish agenda, as a doctor, my duties have expanded to include preventing the disease, ensuring accurate diagnosis and treatment, as well as combating misinformation and providing accurate information.

[118
119
62

post-35747","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-pandemic","tag-pandemic","post_template-post-1","pmpro-has-access
["GET
